To commemorate 40 years of service, Age Concern Hampshire kicked off its Ruby anniversary celebrations in Petersfield last month.
At the charity’s ‘Hospital to Home’ service at Petersfield Hospital, Ruby, a patient of the service, marked her 100th birthday - creating a meaningful connection to the ‘Ruby’ theme. The memorable occasion highlighted the invaluable support provided by the charity in ensuring older people receive the care and assistance they need.
In partnership with the Hampshire and Isle of Wight Healthcare NHS Foundation Trust, the ‘Hospital to Home’ service is committed to actively engaging with patients during their hospital stay, offering tailored support and guidance to ensure a smooth, safe, and timely discharge from the wards to their homes. By addressing individual needs and coordinating essential care, the service helps to reduce hospital readmissions and enables older adults to regain their independence with confidence and security.
Throughout the year, Age Concern Hampshire will be hosting a variety of events, bringing together service users, volunteers, supporters, and the wider community. These events will include fundraising activities, awareness campaigns, and celebratory gatherings aimed at acknowledging the contributions of those who have helped the charity thrive over the past four decades.
